Invasive giant pituitary macroadenoma causing Cushing disease; a case report

Introduction: Cushing disease is a rare entity, and among them, giant pituitary adenomas causing rarer. Here we present case of secondary to macroadenoma. Case report: A 41-year-old female with poorly controlled diabetes mellitus hypertension diagnosed for six months presented history unintentional weight loss, polyuria polydipsia the same duration. There was change in her facial appearance increased pigmentation, frontal balding she also noted increase size abdomen. were features proximal muscle weakness. Symptoms hypothyroidism, hypoadrenalism or hyperprolactinemia absent, amenorrhoeic following hysterectomy done year back. Visual impairment, headache, abdominal pain, suggestive internal malignancy, especially lung renal, including pheochromocytoma absent. She denied family similar illness. Her BMI 20.44 kg/m–2. had prominent abdomen, round facies, hirsutism, male pattern hair plethora generalized pigmentation thinning skin. Rest examination normal. Investigations revealed hypokalaemia, hyperglycaemia and, non-suppressed overnight dexamethasone suppression test. elevated ACTH level, high dose test showed suppression. Pituitary imaging MRI 51x 62x 41 mm sellar para mass heterogeneous echogenicity extending right temporal lobe. Further ectopic secretion negative.She underwent transcranial surgery which unsuccessful due intraoperative bleeding, radiotherapy given postoperatively.  Conclusion: macroadenomas are may require therapeutic measures controlling failed surgery.
